Statement of Compliance
Servants Mission Inc. complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex (including pregnancy, sexual orientation, and gender identity). This includes compliance with Section 1557 of the Affordable Care Act, 45 C.F.R. Part 92, and other applicable state and federal laws.
Servants Mission Inc. does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex.
Free Aids and Services for People with Disabilities
Servants Mission Inc. provides the following free aids and services to people with disabilities when needed to communicate effectively with us about our services:
- Qualified sign language interpreters
- Written information in other accessible formats, such as large print, audio, and accessible electronic formats
These aids and services are provided free of charge.

How to File a Complaint
If you believe Servants Mission Inc. has failed to provide appropriate aids, services, or accommodations, or has discriminated in any way covered by Section 1557, you have two paths for filing a complaint:
File directly with Servants Mission
Contact our Civil Rights Coordinator at civilrights@servantsmission.net. We will acknowledge your complaint promptly and work to resolve it.
File with the U.S. Department of Health and Human Services
You can file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR), electronically through the OCR Complaint Portal or by mail, phone, or TDD:
