If you believe Servants Mission may have mishandled protected health information (yours, a family member's, or someone you support), you can report it here. Submission can be fully anonymous.
The agency's formal breach-notification policy and reporter-protections language are awaiting counsel sign-off. File the concern anyway — Compliance reviews every entry. You are not required to give your name. The agency does not retaliate against good-faith reporters; the formal non-retaliation language will replace this notice once counsel has signed off.
