This route is separate from privacy concerns

Use this form for concerns about services, rights, communication, retaliation, or how support is being delivered. Use the privacy concern form only when the concern is about health information being used, shared, or exposed improperly.

Anonymous submissions are allowed. Filing a grievance does not change services by itself; it gives the agency a record to review and respond to through the appropriate workflow.

Keep clinical details out of this form

Describe the concern in plain language. Do not include diagnoses, medications, or treatment details. If the agency needs more information, it will follow up through the right channel.
